1つのDPC参加地域中核病院で、岩手県保健福祉部および電子カルテシステム作成会社の協力を得て、DPC情報、電子カルテ情報、診療報酬明細情報より、脳卒中登録に必要な項目を自動収集して登録票を自動作成するシステムを作成し、主治医判断により登録された脳卒中登録データと照合し、高い診断妥当性と抽出精度の登録票自動作成システムを構築する。
その後、作成した脳卒中登録票自動作成システムを他のDPC参加地域中核病院に導入を進める。
さらに、各病院からの自動作成された登録票を秘匿性と安全性を確保したWebまたはクラウドにアップロードするシステムを構築する。

| Outline of Final Research Achievements |
A comprehensive survey of the Iwate Prefecture Regional Stroke Registry was conducted based on admission dates from fiscal years 2018 to 2023 across 14 hospitals within Iwate Prefecture and 5 hospitals in neighboring prefectures. Furthermore, among all Diagnosis Procedure Combination/Per-Diem Payment System (DPC/PDPS) hospitals with general beds in Iwate Prefecture (14 hospitals) and DPC data-submitting hospitals (12 hospitals) that only submit DPC data, the extraction of stroke patients eligible for registration based on stroke diagnoses recorded in DPC Format 1 showed a sensitivity of 97.5% and a positive predictive value of 91.3% for all DPC/PDPS hospitals combined, and a sensitivity of 84.7% and a positive predictive value of 67.6% for all DPC data-submitting hospitals combined.
These results indicate the feasibility of establishing a stroke onset monitoring system using DPC Format 1.

| Academic Significance and Societal Importance of the Research Achievements |
脳卒中死亡は徐々に減少しているが脳卒中罹患も死亡率と同様に徐々に減少する経時的変化を明らかにしたこと、さらに脳卒中死亡率が全国ワースト1位の岩手県の中でも大きな地域差が存在することを明らかにしたことが本研究の成果の一つである。
脳卒中発症登録の悉皆調査は人的および時間的負担が大きいが、診療報酬請求目的で作成されているDPCデータ様式1の病名から高い精度で急性期脳卒中患者を抽出できることが明らかとなった。DPCデータ様式1より脳卒中発症モニタリングを構築し悉皆調査による脳卒中発症登録を補完する可能性を見いだすことが出来たのが本研究のもう一つの成果である。
