| Outline of Research at the Start |
本研究課題は、制度開始から20年が経過した専門看護師の活動を明確にし、「専門看護師ラダー」および「専門看護師育成ラダー」を検証すると共に専門看護師の評価指標、地域包括ケア時代に専門看護師に求められる役割、必要とされる大学院教育、修了後の継続教育について検討することを目的とする。これらの目的に沿って、文献検討、専門看護師として認定され２回以上更新している専門看護師への面接調査、多様な専門領域・活動の場・経験年数の専門看護師を対象とした質問紙調査および専門看護師教育課程の教育者を対象とした専門家会議を実施する。

| Outline of Final Research Achievements |
This research project aimed to clarify the expanding roles of certified nurse specialists (CNSs) regarding medical and social conditions and examine the evaluation indicators, roles required in the community-based integrated care system, and education that should be strengthened for CNSs. We found that the roles of CNSs were indicated by activities including “comprehensive assessment”, “designing interventions based on evidence and expertise”, “creating and implementing necessary nursing care and systems”, and “reflecting and evaluating” and by goals including “providing advanced care for all patients (recommendations for healthcare policies, meaningful organizational change)”, “optimizing the patient’s self-care ability to continue their best possible life (patient outcome)”, “independently providing quality care (staff education)”, and “empowering the patient, family, and medical staff”. These results suggested evaluation indicators and education targets for CNSs.

| Academic Significance and Societal Importance of the Research Achievements |
専門看護師の活動や評価を明確に示すことは、診療報酬に結び付きやすい認定看護師や、保健師助産師看護師法で明記されている特定行為などより難しい。専門看護師は、大学院での専門看護師教育を基盤に自ら必要な知識や技術を学び、新たな活動を生み出しているが、その活動を明確にした研究は少ない。また、社会情勢の変化のなかで求められる専門看護師の役割や強化すべき大学院教育・継続教育を明確にすることは社会的意義がある。
