| Outline of Research at the Start |
慢性心不全は主として高齢者の疾患であり、高齢化のさらなる進展により心不全患者の顕著な増加が見込まれている。心不全の緩和ケアについては十分に実施されているとは言い難く、医療従事者を対象とした教育が必要であるが、教育ニーズや教育効果を検討した報告はなく、研修プログラムについてもその内容は確立していない。
本研究では、診断時からの心不全緩和ケアの普及へ向けた研修プログラムの効果を検討することを目的として、1）循環器病棟に勤務する看護師を対象に緩和ケアに関する教育ニーズを明らかにする、2）心不全の疾患特性を踏まえた緩和ケアについて看護師を対象とした研修プログラムを実施、その効果を検証する。

| Outline of Final Research Achievements |
This study examined the effectiveness of palliative care seminar regarding heart failure for nurses. The intervention groups received a ninety-minutes online seminar about concept of palliative care for heart failure, symptom management, and decision-making, and so on using handout. The control groups received the same handout by mail.
Participant demographics were mean age 39.95 years and nursing experience 16.94 years. The effectiveness of the current seminar regarding heart failure were shown that knowledge score of palliative care increased in both groups after the seminar or reading handout. Additionally, difficulty score of palliative care decreased after one month in both groups.

| Academic Significance and Societal Importance of the Research Achievements |
緩和ケアを必要とする人の3人に2人は心臓、肺、肝臓、腎臓、脳など慢性疾患の非がん疾患であるとされている。わが国でも高齢化の進展を背景に非がん疾患の緩和ケアのニーズが高まっている。2018年 4月の診療報酬改定により緩和ケアの対象疾患に末期心不全が追加されたが、予後予測が困難であること、患者・家族と医療者とのコミュニケーション不足などにより適切な緩和ケアが提供されているとは言い難い現況にある。医療チームの一員として、看護師が心不全の臨床経過や予後、アドバンス・ケア・プランニングについて精通した上で、適切な症状マネジメントを行うことは、心不全の診断時からの緩和ケアにおいて重要である。
