全ライフステージにおいて、「歯・口腔の健康格差」の縮小を図ることは、地域歯科保健の大きな課題であるが、成人期以降の「歯・口腔の健康」の地域差に関する研究は少なく、その可視化は十分になされていない。本研究では「歯・口腔の健康寿命」評価法を開発し、「歯・口腔の健康寿命」の地域差を明らかにすることを目的とする。
評価法の開発にあたっては、「健康寿命」の算出でも用いられているサリバン法を用いて、咀嚼に着目した「歯・口腔の健康寿命」評価法を考案する。関連する政府統計データを活用し、都道府県レベルでの「歯・口腔の健康寿命」を求め、GIS（地理情報システム）による地域差の可視化を図る。

| Outline of Final Research Achievements |
This study developed and validated an evaluation indicator to assess disparities in adult and elderly oral health. The age-adjusted proportion of individuals aged 40 and over with 19 or fewer remaining teeth was a valid, practical, and comparable across regions with differing aging rates. Furthermore, two nationwide surveys targeted facilities for people with disabilities and long-term care health facilities for older adults. The results revealed that although daily oral care was commonly provided by facility staff, the regular implementation of dental checkups and oral health guidance by dental professionals was insufficient, with significant regional disparities. These findings underscore the importance of integrating standardized oral health indicators into policy frameworks and enhancing service delivery through stronger collaboration with dental professionals. The findings contribute to evidence-based policy development to reduce oral health disparities.

本研究は、成人・高齢期における歯・口腔の健康格差を評価する標準的指標として「40歳以上で19歯以下の者の割合（年齢調整値）」の妥当性を示し、学術的に有意な指標開発の一助となった点に意義がある。また、障害児者施設および老健施設に対する全国調査により、歯科専門職による定期的な歯科保健サービスの不足や地域差の存在を明らかにしたことは、社会的に支援が必要な集団への歯科保健サービスの現状と課題を可視化し、政策形成に資する実証的エビデンスを提供した点で、社会的意義も大きい。
