本研究は,在宅移行期に焦点をあて,治療段階から在宅療養へと切れ目なく納得の概念を基盤にして,在宅移行期に特有な患者・家族が抱える苦悩や問題,また在宅療養を開始した患者・家族が抱えている問題,さらには医療連携に関わる他職種が認識している問題や課題を明確化し,これらから在宅移行期における高齢進行がん患者の納得を支援する看護ケアモデルを構築することを目指す.

| Outline of Final Research Achievements |
【To develop a nursing care model to support the satisfaction of elderly patients with advanced cancer during the transition to home care】, we reviewed the current situation at domestic and international facilities through literature reviews and submitted a review article. Additionally, we conducted qualitative research on the actual conditions of home care and presented the results at a conference. Incorporating these research findings, we developed a draft nursing care model titled ‘A Nursing Care Model to Support the Satisfaction of Elderly Patients with Advanced Cancer During the Transition to Home Care.’ Subsequently, we reviewed the nursing care model in an expert meeting to adapt it to the clinical setting and refined the model. Through these discussions, it was inferred that for supporting patient satisfaction during the home care transition period, it is important to establish a model centred on interactive communication between hospital and home care settings.

| Academic Significance and Societal Importance of the Research Achievements |
1．本研究は,在宅移行期の高齢進行がん患者の納得を支援する看護ケアモデルを構築するために,施設と在宅の双方向から包括的に捉える所に特徴がある.特に,パラダイム・シフトしていく社会情勢と県下の医療状況を捉え,ケアモデルを構築する所に独自性がある看護ケアモデルが構築できたことに学術的な意義があると考える．
2．現在,高齢がん患者の研究は数少ない.本研究の推進は社会的貢献度が高く,高齢がん患者の看護に関する新しい知見の蓄積に貢献でき,学術的独創性が高いと考える.また,意思決定の質となる納得への着目は真の高齢がん患者のQOL向上に貢献する視点であると考える.
