| Outline of Research at the Start |
がん治療を受ける患者・家族は、自宅等で体調が急変した時の対応が最も不安だとしている。また、がん治療を担う多職種も在宅療養中に急変した際、常時対応可能な場所の存在が治療を継続するために不可欠であるとしている。一方、地域でがん治療を担う医師は担当患者への24時間対応が困難であることや急変時の対応が課題であるとしている。そこで、がん治療を受ける患者・家族が住み慣れた地域で安心して自分らしく療養生活を送るためには、急変時対応に関する地域連携の仕組みづくりが必要であると考えた。
本研究の目的は、急変時対応の現状と課題を明らかにし、その対応を支える地域連携モデルを構築し、そのあり方を検討することである。

| Outline of Final Research Achievements |
A survey of multidisciplinary professionals involved in responding to emergencies at home for patients undergoing cancer treatment and their families revealed challenges in emergency response. Those challenges included a "lack of coordination with professionals responsible for emergency medical care, cancer treatment, and home care" and a "lack of an educational support system for visiting care workers to respond to emergencies." Additionally, a survey of patients identified issues such as "not knowing what symptoms constitute emergencies" and "not knowing what to do if a patient's condition suddenly changes after clinic hours." These results suggest the need for mutual understanding and human resource development among multiple professionals, including caregivers, engaged in responding to sudden changes at home for patients undergoing cancer treatment and their families. Such efforts involve promoting collaboration and providing educational support for patients and their families.

| Academic Significance and Societal Importance of the Research Achievements |
本研究の成果は、がん治療を受けながら在宅で療養生活を送るがん患者・家族の急変時対応を実現するための仕組みの構築に活用できると考える。その仕組みが構築されることで、がん治療を受ける患者・家族が住み慣れた地域で安心して自分らしく療養生活を送ることができることや在宅でがん治療を受けることの意思決定を促進するきっかけになり得る点で意義があると考える。また国は医療と介護の連携により、高齢者が住み慣れた地域で最期まで生活することができるような支援の一つに「急変時対応」を掲げ、医療・介護関係者の資質向上や相互理解による連携促進を目指しており、今回得られた知見は医療と介護の連携深化の機会にもなると考える。
