| Outline of Research at the Start |
本研究の目的は、手術室看護師の安全行動を評価する尺度を開発することである。まず手術室看護師の独自的・専門的な行動に焦点をあて概念分析を行い、どのような行動が事故を防ぐ為に必要か明らかにし、安全行動尺度を開発する。尺度開発により、過去の事例を再発させない、また今後起こりうる事故を想定し事故につながりうる行動を評価できれば、未然の事故防止につながり、安全な手術医療の提供が期待できる。

| Outline of Final Research Achievements |
With the aim of developing a scale to visualize and evaluate patient safety behaviors of operating room nurses, a literature review was conducted to extract seven key domains: patient monitoring, aseptic technique, positioning, prevention of skin injuries, prevention of retained surgical items, specimen handling, and operating room environment management. Based on these domains, a draft questionnaire consisting of 59 items was developed. Results from the first pilot study suggested that the scale might not adequately capture the actual performance of safety behaviors. Therefore, based on prior research, the concept of risk-taking behavior was newly introduced to revise the content to better reflect intentional and unintentional deviations from standard practices. In the second pilot study, item wording and redundancy were reviewed and refined, resulting in the completion of a preliminary scale comprising 52 items.

| Academic Significance and Societal Importance of the Research Achievements |
本研究は、手術室看護師の医療安全行動を可視化・評価するための尺度を開発した点で、学術的・社会的に大きな意義を有する。従来の安全行動尺度は一般病棟の看護師を対象としたものが多く、手術室に特化したものは存在しなかった。本研究では、文献検討により7つの構成概念を抽出し、実務に即した行動項目を整理したうえで、リスクテイキング行動の視点を加味し、より実態に即した行動評価が可能となる尺度を構築した。また、2回のパイロット調査を通じて項目の精査と修正を重ねたことで、実用性と妥当性の高い予備尺度を完成させた。今後尺度を開発することで、患者安全と医療の質の向上にも貢献できると考える。
