Outline of Research at the Start

AMG510の耐性機序の解明、さらに耐性克服治療の開発につなげることが目的。研究方法としてKRAS変異陽性でAMG510感受性肺癌細胞株H23及び同耐性株H23ARC10を用いて耐性の原因遺伝子の探索を行う。すでに網羅的な遺伝子解析によって耐性株でMET遺伝子増幅の出現を確認した(未発表データ)。現在MET遺伝子増幅が同剤への耐性をもたらすか検証中である。siRNAによるMETの選択阻害やMET阻害剤を用いた研究、In vivoでのMET阻害剤等による耐性克服治療の検討を行いたい。またKRAS変異陽性患者由来の腫瘍組織を用いてAMG510の耐性とMET遺伝子の増幅との関連も評価する予定。

Outline of Final Research Achievements

To elucidate the resistance mechanism of the KRAS inhibitor sotorasib, we conducted an exploratory study using sotorasib-resistant clones of the KRAS mutation-positive lung cancer cell lines H23 and H2122, which revealed two resistance mechanisms: 1) MET gene amplification and 2) EGFR abnormal activation dependent on suppression of PTPRR (Protein Tyrosine Phosphatase Receptor R) gene expression were identified. Mouse model studies using these resistant cells showed that the combination of MET or EGFR inhibitors was useful in overcoming resistance to sotorasib. In a retrospective study of patients with KRAS G12C mutation-positive lung cancer, the association between PTPRR protein expression and progression-free survival with sotorasib therapy was poor [hazard ratio 3.27] in the low-expression group.

Academic Significance and Societal Importance of the Research Achievements

KRAS遺伝子変異陽性肺癌症例にとってソトラシブは有益な治療薬であるが、その抗腫瘍効果は限定的であるためより良い治療法が求められていた。今回の研究成果は腫瘍組織のMET遺伝子やPTPRR蛋白を調べることでソトラシブ治療の効果が予測できる可能性が示された。そしてもし効果が乏しいと予想された場合にはMET阻害剤やEGFR阻害剤との併用治療が有用であることを示唆するものであり、将来のより良い肺癌治療の開発につながると期待される。
