Outline of Research at the Start

根治手術を予定するcStageI-III食道癌患者を対象とした、単施設、無作為化比較対象試験である。前方視的に60名を干渉電流型低周波刺激治療機器使用群と非使用群に振り分ける。頚部郭清を行う患者の割合のみ、反回神経麻痺に与える影響を考慮し、均等に振り分けられるように無作為化する。被験機器の適応、効果を評価する介入研究である。
主要目的は、術後嚥下リハビリテーションとして被験機器を周術期に使用することで、術後肺炎を予防できるかを評価する。副次目的は、嚥下機能を改善する効果が得られるかを検討する。また、術後嚥下機能と肺炎との関連性も評価する。

Outline of Final Research Achievements

This study aimed to develop a novel rehabilitation method using interferential current stimulation to prevent silent aspiration after esophagectomy. Although the initial plan was to conduct a randomized controlled trial, it became difficult to implement due to the impact of COVID-19. As an alternative, a prospective observational study was conducted to evaluate perioperative swallowing function using videofluoroscopic swallowing study and cough test. Among the measured parameters, postoperative deterioration was observed in hyoid bone movement distance, pharyngeal transit time, and Penetration-Aspiration Scale score. While no clear associations were found between these parameters and aspiration pneumonia, older age and preoperative pharyngeal residue were suggested as potential risk factors. This observational study provides foundational data for future intervention trials.

Academic Significance and Societal Importance of the Research Achievements

本研究は、最小侵襲食道切除術後の嚥下機能低下をVFSSと咳テストにより定量的に評価し、嚥下障害の発生メカニズムやリスク因子を明らかにした点で学術的意義が高い。特に舌骨移動や咽頭通過時間など、具体的な機能指標の変化が示されたことは、今後のリハビリ介入設計に資する。社会的には、高齢者や術前にリスクを有する患者に対して、早期介入による誤嚥性肺炎の予防が期待され、術後回復や医療資源の効率的活用にも貢献する。
