Outline of Research at the Start

腹部大動脈瘤は下肢の動脈瘤も含め、動脈の分岐点で乱流の生じやすい部位に好発し、腹部大動脈瘤は多くは大動脈分岐上、総腸骨動脈瘤や総大腿動脈瘤も同様に分岐直上にできる。一方、直線状になっている外腸骨動脈や浅大腿動脈の瘤化はほとんどない。本研究の目的は、大動脈瘤抑制のための血流改変モデルを作製解析することで腹部大動脈瘤形成および進展のメカニズムを明らかにし、その上で新たな血管病の治療戦略を呈示することにある。

Outline of Final Research Achievements

The ultimate tensile strength (UTS) of wet normal porcine aortas (PTA) was higher than that of human aneurysmal arteries (HA), while the porosity from dried samples was significantly greater in HA. However, no significant correlation was found between UTS and porosity. To assess their relationship using the same sample, we developed a method enabling tensile testing while visualizing aortic internal structures. Assuming rupture occurs at the boundaries of calcifications or voids, we tested contrast-enhance imaging and found Iomeron effective for structural visualization. The contrast agent caused no noticeable degradation, and allowed imaging of calcification, voids, soft tissue, and adventitial vasa vasorum in wet samples. A CT-compatible tensile testing device is under development, and we plan to examine whether rupture occurs at these structural boundaries.

Academic Significance and Societal Importance of the Research Achievements

本研究は、動脈瘤破裂のメカニズム解明に向け、石灰化や空隙といった構造的異常と破断との関係を、造影剤を用いた可視化技術とCT内引張試験により直接的に検証する新たな手法を確立した点で学術的意義が大きい。従来困難であった生体に近い状態での観察が可能となり、動脈瘤の破裂リスク評価法の革新につながる。また、社会的には、破裂予測の精度向上により、不要な治療の回避や適切な手術時期の選択が可能となり、患者の安全性向上や医療費の削減にも貢献し得る。
