本研究では、前立腺がんでIMRTを受ける患者のQOL向上に資する目的で、適正な膀胱内尿量貯留や排便・排ガスの状態を可能にし、照射位置の再現性向上に有用な看護支援プロトコールを開発する。
具体的には、照射位置再現の困難性をアセスメントするための「問題予測アセスメントツール」及び、患者自身が膀胱容量や排便・排ガスの状態を調整するために生活状況等を記載することで必要な支援を提示する「セルフマネジメント支援ツール」を作成する。その後、2つのツールを包含した『照射位置の再現性向上のための看護支援プロトコール』を作成し、効果の検証を行う。

| Outline of Final Research Achievements |
To improve the QOL for patients with prostate cancer who are undergoing IMRT, this study was conducted with the aim of developing nursing support procedures that would contribute to increasing the replicability of the irradiation position. Challenges related to replication of the irradiation position were identified with medical professionals involved in the treatment of patients with prostate cancer undergoing IMRT. Furthermore, written surveys were administered to nurses in order to extract information on the current situation, measures taken, the realities of nursing, and challenges pertaining to urine retention, bowel movements, and flatus. Subsequently, Radiation Therapy Notes were created as a tool to support the self-management of urine retention and bowel control in patients with prostate cancer undergoing IMRT. Trial usage of the tool was conducted to assess the tool’s ease of use and usefulness.

| Academic Significance and Societal Importance of the Research Achievements |
前立腺がんIMRTにおける照射位置の再現性向上については、医師や診療放射線技師が中心となり、各施設で独自に、経験的に行われてきたが、治療室への入室後に畜尿不足が明らかになり、適正な膀胱容量が得られるまで待機となったり、ネラトンカテーテルによる脱気の処置を受けたりと、患者のQOLが損なわれるケースが多く経験されてきた。本研究で作成した「治療ノート」の使用によるセルフマネジメント支援により、一定の質を担保した看護の提供が可能となり、患者のQOL向上に寄与すると考える。
