Outline of Research at the Start

位置情報を付加した空間医学データをもとに，疾病の発生との関連が強い地域(クラスター)を検出し，その統計的有意性を判定する方法としてスキャン統計量が提案され，空間疫学研究などの分野で実践され重要なツールになっている。しかし，計算処理の高速化や，複数のクラスターの同時検出など，改善・拡充すべき課題が出てきている。そこで本研究では，多様な形式の空間データから精確に複数のクラスターを検出し，その統計的有意性を適切に判定する統計解析ソフトウェアについて検討する。新たなソフトウェアの開発するとともに，空間疫学研究の大規模な実データを用いた実証研究を行うことでその有効性の評価を行う。

Outline of Annual Research Achievements

位置情報を付加した空間医学データをもとに疾病の発生との関連が強い箇所（クラスター）を検出し、その統計的有意性を判定する方法として、最大尤度比に基づくスキャン統計量が提案され、空間疫学研究や症候サーベイランスなどの分野で重要なツールになっている。本研究では、多様な形式の空間データから精確に複数のクラスターを検出し、その統計的有意性を適切に判定する統計解析ソフトウェアについて検討する。新たなソフトウェアを開発するとともに、空間疫学研究の大規模な実データを用いた実証研究を行うことで、その有効性の評価を行うことを目的としている。
2024年度は、flexible scan法を用いて疾病の時空間集積性を解析可能とするR用パッケージを新たに開発した。これは、一時点の空間データに基づく空間クラスターの同定のみを対象としていたrflexscanパッケージを拡張し、時間的変化を含むデータに対応できるようにしたものである。開発したパッケージは、英国の地方自治体（324地域）における2001年から2010年の慢性閉塞性肺疾患（COPD）入院患者データに適用し、時空間クラスターの検出に成功した。解析の結果、有意な時空間集積性が確認され、最も強いクラスターは喫煙率の高い地域に対応し、10年間にわたり継続していたことが示された。
この成果は、任意形状のクラスターを対象とするflexible scan法の利点を時空間解析にも拡張するものであり、従来は不可能だった持続期間の評価を含む疾病集積性の解析が可能となった。一方で、解析に要する計算時間が大きいという課題も明らかとなり、今後は大規模時空間データへの対応を目指したアルゴリズム改良と計算効率の向上が必要である。

Current Status of Research Progress

Reason

これまでに、flexible scan法に基づく空間クラスター解析のRパッケージを拡張し、時空間クラスターの検出が可能な機能を新たに実装した。2024年度はその成果を用いて、英国の慢性閉塞性肺疾患（COPD）入院データを対象とした応用研究を行い、統計的に有意な時空間集積性を確認し、学会発表も実施した。これにより、技術的には一定の成果を達成しており、解析手法の有用性を実証することができた。
一方で、2023年度以前からの研究成果も含めた一連の成果を、学術論文として公表するには至っておらず、当初の計画に比べると進捗はやや遅れていると判断している。現在、研究代表者を中心に論文構成案の作成や必要な図表の整理を進めており、論文化に向けた準備は整いつつある。また、引き続き共同研究者との連携のもと、実データへの応用やパッケージの改良も継続しており、成果の蓄積は順調に進んでいる。
この遅れを解消するため、研究期間を1年間延長し、次年度にはこれまでの開発内容と応用結果を統合的にまとめた論文を完成させ、学術誌への投稿を行う予定である。また、今後もソフトウェアの改良や応用研究を継続し、実用性の高い解析ツールとして広く活用されることを目指す。

Strategy for Future Research Activity

これまでの研究開発内容と応用例を統合的に整理し、論文としてまとめて学術誌へ投稿する。また、rflexscanパッケージの拡張や、新たに開発してきた複数のクラスターの同時同定法や、時空間クラスターの同定法について計算効率向上にも取り組む。特に、大規模データへの対応を可能にするためのアルゴリズム改良や処理高速化を重点的に行う。地理情報解析や空間疫学の研究者らとは引き続き対面・オンラインでの打合せを行いながら、ソフトウェアの開発と実践的研究を進めていく。
