本研究は転移性脳腫瘍に対する定位放射線治療において①照射時期を考慮し線量勾配が異なる照射法を組み合わせる多段階照射(Multi-shot Irradiation: MSI)と、②腫瘍と正常組織における亜致死的損傷からの回復の差を利用し短期間で高線量を投与する過分割放射線治療を組み合わせた最適な過分割多段階照射(Hyperfractionated Multi-shot Irradiation: Hyper-MSI)の開発、検証を目的とする。

| Outline of Final Research Achievements |
The purpose of this research was to develop and evaluate an optimal hyperfractionated multi-shot irradiation technique (Hyper-MSI) in stereotactic radiotherapy (SRT) in patients with metastatic brain tumors, combining irradiation with different dose gradients and hyperfractionated radiotherapy. Although a dose fractionation for Hyper-MSI using a combination of 5 Gy and 4 Gy per fraction (28 Gy in six fractions, administered twice daily) was developed, safety concerns were raised at this time. Therefore, a pilot study was designed that used a new multi-shot irradiation approach. This approach delivered 18 Gy in three fractions in two separate phases, termed two-session fractionated stereotactic radiotherapy. Following approval by the institutional ethics committee, patient enrollment for this pilot study began, and follow-up observation is ongoing.

| Academic Significance and Societal Importance of the Research Achievements |
SRT における過分割多段階照射や2段階寡分割定位放射線治療は、局所制御・安全性の両面において有利となりうるが、それらに関する研究はほとんど実施されておらず、新規性・独自性・創造性の高い研究といえる。2段階寡分割定位放射線治療のpilot試験の症例登録・経過観察を終了した後に結果を解析する予定であるが、この解析結果が転移性脳腫瘍に対するSRT における最適な照射法・線量分割の開発に有用となりうる。また他疾患に対する多段階照射の応用や、近年発展が目覚ましい免疫療法や分子標的療法と本研究成果を組み合わせることにより新たな治療戦略の開発につながる可能性があり、波及効果は大きいと考えられる。
