Outline of Research at the Start

本研究は、てんかんにおける精神・行動面の合併症(うつ・不安、精神病、発達障害等)につき、脳画像を用いてその脳神経メカニズムの解明を試みる。精神・行動的合併症は有病率が高く、てんかん患者の生活の質に及ぼす影響が大きい一方で、それが生じる脳のメカニズムについては報告が少ない。一般人口における精神・行動障害と同様のメカニズムを持つのか、それとも異なるのかにより、有効な治療法・介入法等についても差異がある可能性があり、これらの解明は治療法の改善点やその後の生活の質改善に寄与することが期待される。本研究では、高解像度脳MRI撮像と最先端の解析技術を用いることで、低侵襲にこれらを明らかにする。

Outline of Annual Research Achievements

2023年度は、前年度に引き続き、白質繊維に基づくてんかんの脳年齢研究と、てんかんの白質ダメージの経時的進行に関する研究を継続した。両者の結果について、2023年9月にダブリンで開催された国際学会(35th International Epilepsy Congress)にて発表した。白質ダメージの進行については、155名の片側側頭葉てんかん患者の拡散テンソル画像を教師なし機械学習を用いて解析し、経時的な疾患進行モデルが算出された。焦点側のuncinate fasciculus (UF)から白質繊維障害が始まり、その後、脳梁、anterior thalamic radiation (ATR)、superior longitudinal fasciculus (SLF)などに広がっていくことが示唆された。この結果を原著論文として、現在国際誌に投稿中である。白質脳年齢研究についても、現在投稿中である。この研究では、257名のてんかん群および429名の健常群の拡散テンソル画像から白質繊維に基づく脳年齢解析を行ったところ、視覚的病変の無い側頭葉てんかんや側頭葉外てんかんで4-5年、海馬硬化を伴う側頭葉てんかんで9.1年の脳年齢上昇が起こっており、概ね既報の脳形態画像に基づく解析結果(Sone D, et al. Mol Psychiatry. 2021)が再現されたが、特発性全般てんかん群では有意な上昇を認めない点が過去の解析結果と異なっていた。更に、心因性非てんかん性発作の白質ネットワークに関する総説論文を新たに出版した(Sone D. Neuropsychiatr Dis Treat. 2023)。
