Outline of Research at the Start

リンパ浮腫は一旦発症すると完治することは難しく,継続的支援が必要となる.特に高齢者の発症及び悪化のリスクは高く,ADL,QOLに大きな影響を及ぼしている.リンパ浮腫ケアは専門的な知識,技術が必要なため,多職種の医療職者がケアの中核を担っている.しかし現状では各職種間の連携は十分とは言えない.そこで,多職種のケアの質を担保し,リンパ浮腫への統一したケアの継続を可能とするには,職種間の連携が可能となるモデルが必要だと考えた.研究目的は,在宅高齢者のリンパ浮腫ケアにおける医療職連携モデルを開発することである.これにより医療職のネットワーク化が実現し,安心して生活できる体制が提供できると考える.

Outline of Final Research Achievements

With the aim of developing a medical professional collaboration model for lymphedema care for elderly people living at home, we clarified the actual care, support, and collaboration among the medical professions involved. As a result, medical professionals who provide lymphedema care had a "lack of human resources and information" and a need for "the use of common collaboration tools." It also became clear that in order to systematize the promotion of medical professional collaboration, it is necessary to promote the disclosure of information on local medical resources and to utilize and improve common collaboration tools, which will facilitate smooth collaboration between professions. Based on this, we devised a medical professional collaboration model based on activity theory to share information between multiple professions, ensure the quality of care in each profession, and enable early response to lymphedema and continued unified care.

Academic Significance and Societal Importance of the Research Achievements

リンパ浮腫ケアを実践する多職種の情報共有を図ることができ,リンパ浮腫を発症した高齢者に対し,個々の職種のケアの質を担保し,リンパ浮腫への早期対応,統一したケアの継続を可能とすると考える.
また,モデルを活用した支援によって,リンパ浮腫ケアにおける医療職のネットワーク化の実現につながり,リンパ浮腫を発症した在宅高齢者に安心して生活できるサポート体制を提供できるのではないかと考える.そのモデルを活用した支援によって,リンパ 浮腫ケアにおける医療職のネットワーク化が実現するのではないかと考える.
