Outline of Research at the Start

本研究では、20年間に亘って蓄積した連続肝生検研究から、DiSH進展を予知するバイオマーカーとなるタンパク質とmRNA・miR・lncRを探索する。探索的で芽生え期の研究提案であるが、肝臓での発現と比較しながら、血清およびエクソソーム中のタンパク質とmRNA・miR・lncRを抽出できることが強みである。抽出された候補分子の中には、肝病理変化を予知するバイオマーカーに加えて、肝病理変化と関連して変動する病態制御因子が含まれることが期待される。これら分子の検証と機能解析は、NASH病態体系を転換し、診断、病理予知、治療を変革させる潜在性を有すると考えられ、次のステップで挑戦的研究 (開拓)に申請したい。

Outline of Final Research Achievements

Our serial liver biopsy and diabetes agents intervention studies demonstrated that hyperglycemia independently contributes to liver fibrosis development in NAFLD. Single-cell RNA sequencing-like analyses showed that genes involved in hepatic sinusoidal endothelial cells (LSEC) are downregulated according to liver fibrosis progression. Genes involved in hypoxia and oxidative stress response were upregulated in central hepatocytes according to A1c elevation. SGLT2 inhibitor treatment for 12 months restored the attenuated expression of the LSEC genes. Metabolomic analyses are ongoing to identify blood biomarkers predicting liver pathological changes.

Academic Significance and Societal Importance of the Research Achievements

非アルコール性脂肪肝炎 (NASH)は肝硬変と肝細胞癌の主要な原因疾患である。肝生検による組織学的評価が診断に必須だが、患者に負担のかかる侵襲が大きく、生検部位による不均一性が課題である。また病理学的予後予測法や治療も確立していない。本研究の最終目標は、肝生検をせずとも肝病理を診断・予知する医療の実現である。本研究で得られた、ヒトNAFLD病理学的進展と関連する細胞クラスターと遺伝子プロファイルから、NAFLD病態の鍵となる細胞クラスターとパスウェイの候補が得られた。これらの意義を、申請者等が樹立したマウス糖尿病脂肪肝炎モデルで検証することで、NAFLDに対する診断・治療標的を明らかにしたい。
