Outline of Research at the Start

腫瘍組織の内部には低酸素領域が存在し、がん放射線治療抵抗性にかかわっている。個別化放射線治療を実現するには、患者毎に異なる腫瘍内低酸素の量をモニターして、放射線治療効果を予測する手法を開発することが必要である。本研究では、『低酸素がん細胞が血中に分泌しする分泌タンパク質』に着目し、（1）当該分泌タンパク質が低酸素がんによって発現・分泌されるメカニズムを解明するとともに、（2）海外共同研究者がもつ抗体作成技術を活用して、当該タンパク質に対する抗体を作成し、当該タンパク質の血中濃度を指標に腫瘍内低酸素の量・放射線抵抗性・患者の予後を予測する「高感度の評価系」を確立する。

Outline of Annual Research Achievements

腫瘍組織の内部には、血管から十分な酸素が供給されない低酸素領域が存在し、がんの悪性形質と治療抵抗性を誘導している。本研究開始前までに申請者らは、低酸素がん細胞が放射線治療を生き残り、酸素・栄養環境の良い血管近傍に浸潤して、がんの再発を引き起こすという『がんの再発メカニズム』を解明してきた。また、がんの治療抵抗性と浸潤・転移能を誘導する遺伝子ネットワークを同定してきた。これら独自の知見を個別化医療の実現に繋げるには、患者毎に異なる“腫瘍内低酸素の量”をモニターして放射線治療効果を予測する系を確立することに加え、低酸素がん細胞の放射線抵抗性に関わる遺伝子を同定し、これを阻害する治療法を確立することが肝要である。そこで本研究で申請者は、海外共同研究者との予備研究で見出してきた『低酸素がん細胞が発現・分泌し、自身と周囲のがん細胞の放射線抵抗性を、それぞれオートクリン的・パラクリン的に誘導する分泌タンパク質』に着目し、以下の３つに大別される国際共同研究を展開した。
1. 低酸素がんがSPINK1を発現・分泌するメカニズム、およびSPINK1ががん細胞の放射線抵抗性を亢進するメカニズムを解明した。
2. SPINK1に対する独自のモノクローナル抗体を作成した。そして血中SPINK1濃度を指標に腫瘍内低酸素の量・放射線抵抗性・患者の予後を予測する「高感度の評価系」を確立した。
3. がん患者の臨床検体を対象に、血中SPINK1濃度とがん患者の予後不良の相関を確認した。
以上の研究を通じ、「血中のSPINK1濃度を指標に腫瘍内低酸素画分と患者の予後不良を予測する検査キット」の開発に繋がる基礎データを取得した。また、SPINK1阻害剤が有効ながん患者を選別し、SPINK1阻害剤と放射線の併用療法を適用する個別化医療の実現に向け、礎を築いた。
