Outline of Research at the Start

紹介元の地域医療機関から小児がん患者の複数の診断用画像情報を事前にリモートで取得し、最先端の人工知能(AI)と非剛体変形技術を活用して患者到着前に治療計画・線量計算を行う、新しい陽子線遠隔治療計画システムの基盤技術の開発を目指す。陽子線治療のメリットが大きいにも関わらず、遠隔地在住の小児がん患者・家族に受診をためらわせている経済的・心理的要因について、医理工連携による負担軽減を図り、照射開始までの期間を短縮することによる治療効果の向上、がん治療の均てん化や医療資源の有効活用につなげる。

Outline of Final Research Achievements

To confirm the feasibility of a hybrid remote radiotherapy planning that could reduce the burden on families of pediatric cancer patients living in distant areas who require proton therapy, we combined multiple image data from past brain tumor patients and attempted to reconstruct the treatment plan. For CT images used in treatment planning, targets were set in the same manner as in the pre-existing proton beam therapy plan, and the pre-existing dose distribution (treatment plan) was reproduced using an established dose distribution reconstruction method. The resulting dose distribution was then compared with the dose-volume histogram (DVH) of the actual treatment plan actually used in clinical practice. The results showed that the quality of the DVH of the reproduced treatment plan was equivalent to that of the clinical treatment plan, and a significant reduction in the time required for re-planning was confirmed, suggesting the potential for future clinical application.

Academic Significance and Societal Importance of the Research Achievements

小児がん陽子線治療は成長障害や二次がんリスク低減等のメリットが大きいが、治療用固定具作成・治療計画用CT撮像後、照射開始までに数週間の待機時間が必要で、移動や宿泊にかかる費用等の経済毒性のみならず、職場との勤務調整や患者付き添い、きょうだいや高齢家族の世話等の要因が、遠隔地在住の患者にとって受診をためらわせる心理的負担となっている。本研究成果により受診前に陽子線治療準備を先行し高品質な治療計画を作成することで、来院回数減と照射開始までの待機期間短縮が可能となれば、遠隔地在住の小児がん患者家族の経済的・心理的負担が軽減され、がん治療の均てん化や医療資源の有効活用につながるものと期待される。
