Outline of Research at the Start

まず、短時間で撮像可能なMR灌流強調像プロトコールを開発し、プロトコールの最適化および観察者間一致率の高い解析法の構築を行う。次に、開発された撮像プロトコールを急性期脳梗塞患者に用い、虚血コアおよびペナンブラ領域の推定を行う。併行して、無症候性主幹動脈高度狭窄ないし閉塞患者の撮像を行い、開発されたプロトコールの妥当性および他の灌流強調像との相関を検討する事により、新たな脳灌流形態評価法の確立を図る。

Outline of Final Research Achievements

This study aimed to establish a rapid and non-invasive diagnostic method for acute stroke and cerebrovascular diseases. DWI with six b-values was employed to evaluate patients with acute ischemic stroke. From the IVIM model, bi-exponential fitting was applied to extract perfusion fraction (f) and diffusion coefficient (D) values. The results demonstrated high interobserver agreement in both f and D values, indicating the reliability of the IVIM approach. Furthermore, the study investigated the relationship between IVIM-derived f values and cerebral blood flow ratio (CBFR), obtained from SPECT imaging. A significant negative correlation between f and CBFR was found in patients with moyamoya disease (MMD), suggesting that reduced f values may reflect impaired perfusion. In contrast, no correlation was observed in patients with non-MMD. These findings highlight the potential of IVIM imaging as a useful tool in differentiating pathophysiological mechanisms of cerebrovascular diseases.

Academic Significance and Societal Importance of the Research Achievements

非侵襲的な画像診断は急性期脳梗塞および脳血管障害診断に広く利用されているが、これまでのMR灌流強調画像は撮像時間の長さが短所となっている。社会復帰を可能とするためには、症状発現後できるだけ早く適切な治療を受けることが重要であり、迅速な治療開始のためには早期のより正確な画像診断が望まれる。MR拡散強調像、およびみかけの拡散係数(ADC値)は急性期梗塞巣の同定・診断において中心的な役割を担っており、短時間で撮像可能で観察者一致率が高い本研究の灌流強調像プロトコール撮像により、高齢化の進行により増加し続けている脳梗塞患者の治療前診断能向上に寄与する事が期待される。
