| Outline of Research at the Start |
原発性アルドステロン症は、その成因によりアルドステロン産生腺腫と副腎過形成に大きく分類される。両者は治療方針および重症度が異なるため病型の鑑別は重要であり、原発性アルドステロン症におけるさらなる治療成績の向上には早期診断・治療を可能とする医療基盤の確立が求められている。本研究では機械学習を用いて、原発性アルドステロン症においてステロイド産生の多様性・不均一性はなぜ生じるのか、そしてどのような病態生理学的意義を有するかを解明する。さらに、血液検体のステロイドプロファイリングと腫瘍組織の統合オミクス解析の結果を踏まえて、原発性アルドステロン症における個別化医療基盤の確立を目指す。

| Outline of Final Research Achievements |
This study aimed to elucidate the pathophysiology of primary aldosteronism (PA) and improve diagnostic accuracy by utilizing machine learning and integrative omics analyses. By employing cutting-edge technologies such as single-cell RNA sequencing, spatial transcriptomics, and imaging mass spectrometry, we revealed the diversity and heterogeneity of steroid-producing cells in adrenocortical tumors, and clarified the intratumoral differentiation process and disease progression mechanisms. Furthermore, machine learning based integration with circulating steroid profiles enabled the identification of biomarkers associated with comorbidities. This study contributes to the development of a next-generation medical platform for the early diagnosis and treatment of PA and other adrenal disorders.

| Academic Significance and Societal Importance of the Research Achievements |
本研究は、機械学習を用いることで、原発性アルドステロン症の病態進展における細胞・分子レベルの不均一性を、単一細胞解析や空間トランスクリプトミクスなど先端的手法により可視化し、分子病態解明を行った点で学術的に大きな意義がある。これにより、従来の画一的な診断・治療から脱却し、病態に応じた精密医療への展開が可能となる。さらに、高血圧の原因疾患として重要なPAを早期に診断・適切に治療することで、脳心血管イベントの抑制や生活習慣病の管理にも寄与し、患者のQOL向上および医療経済への貢献といった社会的意義も期待される。
