骨軟部感染症患者に対するナノポアシーケンサーMinION MK1C による細菌同定および薬剤耐性の診断精度を明らかにする。また骨軟部感染患者において、ナノポアシーケンサーによる感染経路の解明が可能であるかについて明らかにする。薬剤耐性遺伝子を用い精度で同定することが可能であれば、難治性となることが多い細菌培養陰性症例における抗菌薬の選択への応用と治療成績の改善が見込まれる。また、感染経路について解析することで、感染巣における感染治療後の再感染の防止に貢献することが期待される。

| Outline of Final Research Achievements |
We evaluated the diagnostic performance of the nanopore sequencer MinION MK1c for periprosthetic joint infection (PJI). A total of 61 culture-positive PJI cases from July 2020 to May 2022 were analyzed. Bacterial identification using MinION was compared with conventional culture and 16S rRNA PCR. MinION detected 74.5% of culture-identified genera and additionally identified bacteria not detected by culture in 47.5% of cases. While culture revealed polymicrobial infection in only 3 cases, MinION suggested it in 24 cases. Among 16S PCR-positive cases, MinION showed a detection rate of 81.6%. These findings suggest that MinION is useful not only for culture-positive cases but also for culture-negative and polymicrobial infections.

本研究は、ナノポアシーケンサーMinION MK1Cが、従来の細菌培養やPCRでは検出困難な病原菌や混合感染の同定に有用であることを示した。特に培養陰性例や多菌種感染の診断精度向上が期待され、早期診断・適切な抗菌薬選択に貢献する。さらに、感染経路の同定により再発防止や感染対策の強化が可能となり、整形外科領域における感染症管理の新たな診断基盤を提供する点で学術的・社会的意義は大きい。
