Outline of Research at the Start

本研究計画は腱板修復モデルラットに対し,体外衝撃波治療(ESWT)の腱板修復促進効果を評価することが目的である.腱板断裂とその修復を行ったラットにESWTを行い,治療後の腱板修復部の組織学的,組織修復関連遺伝子・タンパク発現の差異を比較することでESWTの組織修復促進効果を評価する.この研究結果は術後比較的長期間の運動制限を課される腱板断裂治療の期間短縮を可能とする一助となり得る.

Outline of Final Research Achievements

This study established a rat model of rotator cuff repair with a left supraspinatus tendon rupture under general anesthesia, followed by repair at 3 weeks. Rats were randomized into ESWT and control groups. The ESWT group received treatment one week post-repair, and healing was assessed histologically at 4 weeks. Results showed a significantly lower ratio of cell numbers at the repair site (surgical/contralateral) in the ESWT group. Furthermore, qRT-PCR revealed significantly upregulated CCN2 and SCX mRNA expression in the repaired tendons of the ESWT group. These findings suggest ESWT as a potential adjunctive therapy to promote tendon healing in rotator cuff repair, possibly via chondrogenic cell activation.

Academic Significance and Societal Importance of the Research Achievements

本研究は通常,物理的修復後の生態学的修復までに数ヶ月の長期間を要する腱断裂の治療において,筋骨格系疾患の治療に既に使用されている既存の技術(ESWT)の応用を調査した研究である.本研究の結果から,ESWTは極めて簡便かつ非侵襲的に腱板修復後の生態学的修復までの期間を短縮しうる可能性が示唆され,これにより,再手術の可能性やリハビリ期間の短縮など医療費の抑制にも繋がりうる結果であった.
