Outline of Research at the Start

革新的低侵襲癌治療法である重粒子線治療を行う腎細胞癌患者および前立腺癌患者の末梢血中におけるGPI-80を含む免疫パラメータ発現と免疫細胞機能の変化を解析し、治療効果との関連を評価する。さらに重粒子線治療における最適な患者選択および治療効果を予測するマーカーの確立と治療効果増強に関わる免疫学的パラメータを探索し、より効果的な重粒子線治療と免疫療法の併用による癌治療戦略の開発を目的とする。

Outline of Final Research Achievements

Heavy ion therapy for localized prostate cancer has been covered by insurance and is expected to be effective; however, there are very few reports on its impact on the immune system. We conducted blood sampling during and six months after heavy ion therapy in patients with localized prostate cancer, and evaluated multiple leukocyte subsets using flow cytometry. Principal component analysis of 29 leukocyte subsets revealed that the second principal component most clearly distinguished between the treatment and post-treatment phases. This second principal component was associated with antigen presentation responses, including markers such as CD74, CXCL10, HLA-DR, and EGR1, as identified by factor analysis.

Academic Significance and Societal Importance of the Research Achievements

本検討で重粒子治療による免疫学的動態として、抗原提示応答を上昇させることが示された。現状、前立腺癌に対する免疫学的治療は臨床応用される段階にはないが、重粒子治療と免疫治療の併用が治療効果を示す可能性を示唆する所見である。また、今後の検討として、前立腺癌重粒子治療と予後の関連を検討することにより、重粒子治療の効果予測マーカー探索に応用可能であることや、免疫治療が臨床応用されている腎癌に対しても同様に抗原提示応答を上昇させるのかを検討することにょり、他癌種との共通性や異質性を検討することが可能である。
