本研究では、泌尿器がん患者の中でも、術前後で生活スタイルや日常生活機能が大きく変化する膀胱全摘除術を受ける患者に着目し、手術前後のHRQOLとIADLを経時的に評価する。特に、術後にHRQOLやIADLが損なわれる患者の因子についての基盤データを作成し、術前から術後にかけての早期介入（リハビリ、公共サービス等）による改善の可能性を探る。更に、これらのPROの結果を積極的に日常臨床に利活用し、術前の意思決定をサポートできる情報提供ツールを開発する。

| Outline of Final Research Achievements |
This study aimed to clarify changes in health-related quality of life (HRQOL) from preoperative to postoperative periods in patients with bladder cancer scheduled for radical cystectomy, and to develop an information provision tool to support decision-making. To establish the foundational data for the information tool, QOL data were collected through a multicenter prospective cohort study. These foundational data are planned to be utilized as materials for Shared Decision Making to inform preoperative surgical procedure selection. Additionally, a nomogram for predicting severe complications based on preoperative factors for cystectomy was developed and made publicly available on a website (https://jprediction.jp/) for use by patients and healthcare professionals.

| Academic Significance and Societal Importance of the Research Achievements |
本研究では、膀胱全摘術を予定する膀胱がん患者を対象に、術前から術後にかけての健康関連QOL（HRQOL）の変化を明らかにし、意思決定支援に資する情報提供ツールの開発を目的とした。本研究から得られた基盤データは、今後、術前の術式選択に資するShared Decision Makingの資料として活用できる。また、膀胱全摘術前因子に基づく重度合併症予測ノモグラムは、患者や医療従事者が活用可能なWebサイト（https://jprediction.jp/）として公開されており、患者に対する術前情報提供の質を向上させ、実効的な意思決定支援の実現に貢献する。
