非アルコール性脂肪性肝炎(NASH)の食餌誘導性モデルマウスを用いて、幼若時に抗菌薬暴露による腸内環境の撹乱を行い、脂肪肝炎の発症への影響を調べる。幼若時に抗菌薬暴露したマウスに成体になってから高脂肪食を与えると、肝障害が急速に悪化する個体が出現するので、肝傷害が進展する個体とそうならない個体の腸内菌叢の相違点、盲腸内の有機酸や胆汁酸の組成の相違、腸管のバリア機能等を調べ、脂肪肝炎の発症・進展のリスクと腸内環境との関連性を明らかにする。

| Outline of Final Research Achievements |
We used a diet-induced steatohepatitis model in which mice were fed a western diet from the age of 8 weeks to examine the effects of exposure to the antibiotic ampicillin (AMP) during early childhood on hepatitis level, gut microbiota composition. Mice fed a western diet had elevated NAS scores, a sign of fatty liver disease, and some of AMP-exposed mice showed higher NAS score and advanced fibrosis. AMP exposure significantly reduced the diversity of the fecal microbiota at 4 weeks of age, and also affected the composition of the fecal microbiota after the ingestion of a western diet. There was a significant negative correlation between the total score of steatohepatitis and the diversity of the fecal microbiota at 4 weeks of age, as well as the amount of Bifidobacterium in the feces. These results suggested that exposure to AMP during early life induce gut dysbiosis, which probably continue for long time, and affects the progression of diet-induced steatohepatitis.

| Academic Significance and Societal Importance of the Research Achievements |
食餌誘導性脂肪肝炎モデルマウスを用いて、幼若期の抗菌薬アンピシリンへの暴露が腸内環境のdysbiosisを引き起こし、成体になってからのウェスタンダイエット摂取による脂肪肝炎の発症や程度に影響すること、腸内細菌叢が確立する以前の幼若期の抗菌薬暴露が腸内細菌叢の多様性や特定菌種の多寡に関り、その影響が継続する可能性があることを示した。抗菌薬暴露により欠落または異常増加した腸内菌が脂肪肝炎の進展や抑制に関わる機序が明らかになれば、そこを標的とした新たな予防策の開発につながる。感染症治療での抗菌薬使用は必須だが、その後の生涯にわたる腸内環境に対するデメリットを軽減できるものと考える。
