Outline of Research at the Start

現在の通常のCTは臥位で撮影するため、ヒトの日常生活の状態(立位/座位) を反映しておらず、立位/座位での様々な病態を3次元的に画像化できていなかった。そこで我々は産学共同で320列立位CTを開発し、当施設に世界第1号機を導入した。これまでの初期検討により、立位と臥位で解剖学的構造や各種病態はかなり異なる事が分かった。この初期結果をふまえ、本研究では、新たな診断機器である立位CTを用いて、立位荷重下でより顕著となる加齢性変化や各種病態を画像的に解明しつつ、立位/座位での新たな疾患早期検出法の確立を目指す。そして立位CTによる早期診断により、超高齢社会における健康寿命の更なる延伸、生活の質の向上を目指す。

Outline of Annual Research Achievements

漏斗胸では立位時に胸郭の前後径が短くなり、心肺症状が悪化する可能性が示唆された(Eur J Cardiothorac Surg. 2024;66:ezae408)。変形性膝関節症において、2次元と、立位CTを用いた3次元の計測を比較すると、下肢アライメントや膝関節線の傾斜に差異があり、特に立位CTを用いた3次元計測の方が正確な評価が可能であることが示唆された(Knee. 2025;53:237-45)。立位四次元CTで肩関節の動作を解析し、報告した(BMC Musculoskelet Disord. 2024;25:589)。立位CTを用いた解析で、加齢や性別によって胸腰椎の回旋能力に有意な差があることが示された(Gait Posture. 2024;114:305-12)。股関節から膝までのCT画像において、骨と筋肉の体積および密度を自動的に評価する深層学習モデルを検証した(Sci Rep. 2025;15:125)。内視鏡下経鼻頭蓋底手術後の脳構造(Sci Rep. 2024;14:20303)、肺野の濃度勾配(Sci Rep. 2024;14:21493)に関する臥位と立位の違いを明らかにした。高齢女性の転倒による股関節骨折リスクは、骨盤の軟部組織の形状や転倒方向により大きく変化するが、立位CTデータの解析により、特に後外側からの転倒では骨折リスクが高く、ヒッププロテクターよりも衝撃吸収性床材が有効であることが示唆された(J Biomech. 2024;172:112199)。立位CTと独自開発のフォースプレートを用いて、静止立位中の下肢骨格の三次元姿勢と床反力ベクトルを同時に計測する新たな手法を開発した(J Biomech. 2025;183:112630)。立位CT(本研究課題)に関する総説を発表した(Jpn J Radiol. 2025, in press)。

引き続き、臨床各科との連携を密にして、様々な疾患における立位での各種病態を画像的に解明していく。また、呼吸機能・嚥下機能・排尿機能・歩行機能含め
た各種運動機能など、立位CT(立位四次元CT)を用いた機能に関する評価も進めていく。
