Outline of Research at the Start

臨床研究では,新規治療が標準治療に比べて有効であるか評価するために,比例ハザードモデルという生存時間解析手法がよく行われる.ここで,比例ハザードモデルに必要な仮定として,比例ハザード性というハザード比が時点に依らず一定である性質が求められる.しかし,実際の臨床研究から得られる生存時間データでは,比例ハザード性を仮定できない場合がある.がん免疫療法の開発で得られる生存時間データは,比例ハザード性が仮定できない代表例といえる.本研究では,がん免疫療法の臨床研究をはじめとした,近年の生存時間解析の諸問題への新展開となり得る,新たな統計解析手法を開発することを目的とする.

Outline of Annual Research Achievements

本年度は,生存時間解析に関する研究プロジェクトとして, [1] 相関のある生存時間データの効果の推定量の開発,[2] 比例ハザード性が成り立たない状況における時間依存性ROC曲線による推定方法の開発,[3] 生存時間データに対する新たなWin統計量の開発を行った.
[1] 生存時間アウトカムの2群比較として用いられるMann-Whitney効果は,2群の生存時間が独立だという仮定のもと,妥当な推定量を計算している.そこで,相関のある2群の比較を行う場合の方法として,生存時間の同時分布にコピュラモデルをあてはめ,Mann-Whitney効果の推定方法を開発した.研究成果を学術論文として投稿し,来年度に学会発表を通じて情報発信する予定である.
[2] 前年度までに,比例ハザード性が仮定できない状況における統計解析手法として,境界内平均生存時間を活用した新たな統計的推測方法を開発している.そこで,これまでの方法論を用いて,比例ハザード性が仮定できない状況における時間依存性ROC曲線の推定方法を開発することとした.そのために,まずは複数の時間依存性ROC曲線の推定方法に関する文献調査を実施し,推定方法に絞ったうえで性能比較を行った.研究成果について,来年度に学会発表を通じて情報発信する予定である.
[3] 複合評価項目の構成要素に優先順位を付けたWin統計量に関して,不完全なフォローアップによる右側打ち切りデータを伴う生存時間アウトカムに適用可能なWin統計量を開発した.研究成果を学術論文として投稿した.

研究成果を学術論文として投稿し,国際学会・国内学会での発表を通じて情報発信につとめた.また,これまでの研究成果をもとに,新たな研究プロジェクトにも着手できた.以上,本年度に目標としていた内容をほぼ達成できたため,おおむね順調に進展していると考えている.

Strategy for Future Research Activity

本研究課題の最終年度であるため,分担研究者および共同研究者と協働して,各プロジェクトの研究成果をまとめ,学術論文として投稿する予定である.また,学会発表により研究成果の情報発信も行う.
