Outline of Research at the Start

多発性硬化症(MS)と慢性炎症性脱髄性多発神経炎(CIDP)は、中枢(CNS)と末梢神経(PNS)を侵す脱髄疾患だが、自己抗原は未発見である。MSでは確立したノド抗体はない。私たちは線維走行が一定で絞輪を同定しやすいマウス視神経の組織免疫染色法(TBA)を確立した。本研究では、①既知抗体陰性CNS脱髄患者で視神経TBAにより新規CNSノド抗体・抗原を同定し、臨床像を明らかにする。②IgG3抗120kDCNSノド抗体、CNSとPNSを侵すIgG4抗NF155抗体、PNSのみを侵す抗LGI4抗体を用いた受動免疫・能動免疫・細胞培養モデルでCNSとPNSでの脱髄機序を解明する。

Outline of Final Research Achievements

Multiple sclerosis (MS) shows widespread nodal protein loss beyond the demyelinated areas. We discovered a novel nodal antibody to ATPase Na+/K+ transporting subunit alpha 2/3 (ATP1A2/3) in idiopathic central nervous system inflammatory demyelinating disease (CNS-IDD). By a live cell-based assay for anti-ATP1A2/3 antibody, we found that anti-ATP1A2/3antibody-positive IDD showed higher frequency of juvenile onset, female preponderance, higher EDSS scores, igher frequency of optic nerve involvement, and higher frequency of huge brain and spinal cord lesions than anti-ATP1A2/3 antibody negative MS. Serum glial fibrillary acidic protein levels measured by single molecule array were increased at relapse and progressive phases in anti-ATP1A2/3 antibody-positive IDD. Anti-ATP1A2/3 antibody-positive IDD shows higher frequency of autoimmune trait and HLA-DR4. Anti-ATP1 A2/3 antibody is a nobel marker for seronegative NMOSD/CCPD and progressive MS.

Academic Significance and Societal Importance of the Research Achievements

MSで初期からみられるノドパチーの原因は不明だったが、本研究により自己抗体介在性機序が関わっていることを明らかにできた。特に抗体陽性例は重症化しやすいことから、抗体介在性機序が病態の進行に重要であることがわかった。したがって、抗ATP1A2/3抗体は、少なくともMSの増悪因子としての役割を担っていると考えられた。一方、既知抗体が陰性のMS以外の脱髄疾患は原因が全く不明であった。そのうち、既知抗体陰性のNMOSDやCCPDで抗ATP1A2/3抗体が高率陽性になることから、これらの疾患の発症において、抗ATP1A2/3抗体が病因的役割を担っていることを初めて示すことができた。
