Outline of Research at the Start

本研究では、多様な医療・介護機関で作成される看護サマリーの実態を明らかにし、各機関での看護サマリ―作成に関する実践知を導き出す。これらの結果をもとに、個別最適となる看護サマリー様式を構造化するロジックを開発する。最後は新サマリーを地域で実装する。対象看護職には事前教育プログラを経て介入を行い、実践者からの評価と患者アウトカムによって新サマリーの効果を明らかにする。
