（研究１）慢性疾患を持つAYA世代の女性患者のPCCニーズを明らかにすることを目的に病院の内科外来に受診しているAYA世代の糖尿病、潰瘍性大腸炎、クローン病、リウマチ膠原病をもつAYA世代の女性患者30名に構成的面接調査を行う。
（研究２）内科医師、産科医師、看護師、助産師の多職種連携によるPCCプログラムを開発し検証する。研究１の結果をもとにPCCプログラムを開発し、病院のAYA世代の糖尿病、潰瘍性大腸炎・クローン病、リウマチ膠原病を診療している内科医師と産科医師の各２名および各診療科外来看護師各２名の合計16名に意見を求めて内容を修正し、多職種連携によるPCCプログラムを開発する。
