Outline of Research at the Start

病院から在宅に移行する患者・家族への退院支援が定着しているものの退院後に医療・介護ケアが中断することが少ないとは言えず、ケアの継続性に課題がある。特に高齢脳血管疾患患者は、後遺症による身体・認知機能の低下や再発リスクが高く、安心できる在宅生活につなげるには病棟看護師によるシームレスケアが重要である。本研究の目的は、シームレスケア実践ガイドラインを臨床で運用し、病棟看護師によるガイドラインの評価を行い、シームレスケアのアウトカムとして患者視点での評価を得ることである。
