Outline of Final Research Achievements |
The purpose of this research is to focus on the creation of "life logs" for elderly people with dementia, to establish a methodology to facilitate the collecting of information for these "life logs", to share them among facilities, and encourage collaboration among facilities so that caregivers can support people with dementia in living their own lives on a continual basis. We conducted questionnaire surveys through a multistage sampling method with caregivers and staff members who are in charge of admissions and discharges in facilities covered by long-term care insurance nationwide. From the research results, the following initiatives were suggested: ①the creation of an assessment sheet for "life logs", etc. that can be used within a facility or between facilities, ② the creation of notebooks called "my notebook (provisional name)" written by the elderly people themselves, ③ conducting training to ensure confidentiality, and ④ working on issues through the entire facility.

Academic Significance and Societal Importance of the Research Achievements |
認知症高齢者の生活歴等の情報収集・施設内共有・施設間連携がスムーズにできる方法論を提言できたことは,認知症高齢者に限らず,高齢者が在宅から施設入所そして看取りまで,継続して「その人らしい」ケアを受けることができる.また,認知症特有のBPSD(認知症の行動・心理症状)の原因や対処法を知る手がかりとなったり,転院等(在宅→施設、施設→施設)によるリロケーションダメージを早い段階から軽減できるほか,認知症高齢者とのコミュニケーションをはかるためのツールとなるなど,認知症高齢者ケアの有用な方法論となりうる.
