研究概要

〔研究結果〕
1.進行性核上麻痺(PSP)の大脳皮質の抗tau,ubiquitin,neurofilament抗体による免疫組織化学と光顕Gallyas染色による検討:PSPの11剖検例ののすべてに神経原線維変化(NFT)とGallyas染色で嗜銀性を示すneuropil threadsが検出された.また膠細胞にGallyasと抗tau陽性の細胞質内封入体が見いだされた.
2.透過型電顕検索:twisted tubules(TT)からなるNFTが多数検出された.一部にstraight tubulesからなる特異的なPSP-NFTが見いだされた.
3.Gallyas電顕検索:大脳皮質に出現するGallyas陽性のneuropil threadsはTTが主要構成線維であった.神経突起由来であることが明瞭なものもあるが,不明瞭なものもあった.星状膠細胞のグリア線維束,髄鞘などはGallyas陰性であった.
4.PSPと神経病理学的類似性が近年議論されているcorticonigral degeneration with neuronal achromasia(CND)の大脳皮質を同様の方法で検索した.CNDの嗜銀性封入体は,PSP-NFT,Alzheimer-NFT,あるいはPick bodyとは免疫組織化学的,超微形態的に異なっていた.
〔結論〕
1.PSPの大脳皮質のGallyas陽性のneuropil threadsはTTが主要構成線維であった.
2.PSPの大脳皮質にはPSP-NFTの出現や膠細胞の変性があり、これらの変性過程は生理的老化とは質的に異なる.この変性過程はPSPの痴呆の原因の一つと考えられる.
3.PSPとCNDは細胞病理学的に異なった疾患である.
