研究概要

ヒト慢性B細胞性白血病細胞における白血球分化抗原および免疫グロブリン重鎖(VH)遺伝子発現を日本人患者の白血病細胞を用いて解析した.特に通常ではT細胞抗原とされるCD5抗原の細胞上への発現の有無に注目して,CD5抗原陽性群とCD5抗原陰性群との二群に分けて白血球分化抗原および免疫グロブリン重鎖遺伝子の発現の検索を行った.
CD5抗原は慢性B細胞性白血病26症例中21例に発現されていた.骨髄単球系抗原であるCD13抗原,CD11bの検索を行った.CD13抗原はCD5抗原陰性の全5症例に発現されていたが,CD5抗原陽性群においては1例も発現されていなかった。CD5抗原陰性群ではCD11bを80%とCD5抗原陽性群の11%に比して高頻度に発現していた.他の骨髄系抗原CD33の白血病細胞での発現は,CD5抗原陽性群,陰性群いずれにおいても認めなかった.B細胞分化マーカーであるCD22はCD5抗原陰性群で高頻度に発現していた.他のB細胞分化マーカーであるCD23の発現はCD5抗原陽性群,陰性群両群で同程度であった.CD45RAは白血病B細胞の大多数に発現されていたが,その発現レベルはCD5抗原陰性群で有意に高かった.
ポリメラーゼ連鎖反応増幅法(PCR)を用いて白血病B細胞の発現VH遺伝子を解析した.CD5抗原陽性群はVH4遺伝子メンバーが選択的に使用されていた.一方,CD5抗原陰性群では,主としてVH3遺伝子メンバーが使用されていた.
この慢性B細胞性白血病の解析研究において,CD5抗原陽性群と陰性群での骨髄単球系抗原の発現の差異を見い出した.さらに,白血病B細胞のCD5抗原発現とVH遺伝子利用状況に関連を見い出した.これらの結果は,これまでのCD5抗原の慢性B細胞性白血病の中での発現はB細胞性慢性リンパ球性白血病として異なった疾患概念の目安であるとする報告を支持するものである.
