研究概要

平成6〜8年度に亘る3年間の中華人民共和国佳木斯医学院との大学間協力研究において派遣4名、招へい延べ6名を行い、中国側における研究体制の確立に努力し、中国側症例をも用いて日本側症例と同様に検討した。検索項目は悪性腫瘍におけるS-100蛋白陽性樹状細胞(S-100DC)の出現とp53蛋白を主とする癌(抑制)遺伝子産物発現と予後との相関である。
日本側で得られたデータでは、皮膚の悪性腫瘍の内、扁平上皮癌・ボ-エン病・基底細胞癌について検討し、扁平上皮癌の内、分化度の高い組織内でのS-100DC出現が統計的に有意に高く、基底細胞癌では数が少なく、S-100DCの形も小型であった。ボ-エン病はこの両者の中間的性格を有していた。ついで肝細胞癌については、S-100DC出現は54例中40例に認められ、数の多いものの予後がより良好であり、又、腫瘍の増大とS-100DCの数は密接な関連を有しており、肝癌においてはS-100DCは予後判定の有用なマーカーであることを明らかにした。膀胱癌でも同様の結果が得られた。又、食道癌においても同様にS-100DCの出現数と予後は密接な相関を有していることが示された。p53については、皮脂腺癌において、転移や再発を生じたものでより高頻度に強く陽性となる所見が得られ、食道癌でもp53陽性例の予後は不良であった。このようにS-100DCは予後良好を示唆するマーカーであり、逆にp53は予後不良をあらわすマーカーであることが明らかであった。
腎盂・尿管癌では、98例につき検討した結果、26例(26.54%)にp53過剰発現をみ、又、ヒト乳頭腫ウイルス(HPV)16と18型DNAも32例(32.7%)に検出され、6症例は双方が陽性であった。p53陽性例では、浸潤性非乳頭型で且つ高異型度例に頻度が高く、一方、HPV陽性例では非浸潤性乳頭型症例が統計的に高頻度であった。又、腫瘍に近接した異型上皮部もp53やHPV陽性例が各々73%、66%に見出され、これらは腫瘍細胞の増殖様式を規定するのみでなく、腫瘍化の初期変化にも関与している可能性が示唆された。又、bcl-2蛋白の過剰発現が21例(22.3%)に認められ異型上皮部にも認めたことから、腫瘍化初期において重要な役割をp53蛋白と共に果たしていると思われた。
更に前立腺癌についてのMDM2蛋白、食道癌におけるサイクリンA、膀胱癌におけるサイクリンD1、Rbの各過剰発現についても検討し、いずれも不良な予後と相関することを見出し各々報告した。
一方、中国側症例については、膀胱癌5例、咽頭癌8例、喉頭乳頭癌2例について、S-100DCを検討したデータでは、異型度が低いか、分化度が高い症例ではS-100DCの出現が高異型度や低分化な症例に比し数も多く、形も多彩で大型のものが認められ、日本側で得られたデータと類似の傾向が得られた。更に中国側症例では、96例の膀胱移行上皮癌の内、34例(35.4%)にp53蛋白の過剰発現をみ、18例(18.8%)にS-100DCが多数みられ、12例(12.5%)にHLA-DR陽性細胞を多く認めた。p53陽性例では高異型度のものに陽性率が高かった。又、49例の食道扁平上皮癌では、p53蛋白は21例(42.9%)に陽性であり、S-100蛋白或いはHLA-DR陽性細胞は各々9例(18.4%)、7例(14.3%)において多数認められた。p53はstage IVに高率に陽性であった。これらのデータも日本側症例と類似したものであった。
最後に肉腫に関しては、日本側症例においても、中国側症例においてもS-100DCの出現は極めて乏しく、これらが予後不良を示唆しているものなのか、又、S-100DCの出現機構が癌腫とは基本的に異なっていると解釈するのか今後の検討が必要である。文献上、これに関する報告は極めて乏しい。
