研究概要

本研究では、冠血管に作用する力学的ストレス・代謝的刺激が伝達・処理されて血流が調節される機構についてミクロからマクロに至るまでの階層的システムレベルで理解することを目標にして各研究分担者のグループが学際的に協力して研究を進めた結果以下の研究成果と今後の研究展開のための見通しが得られた。
1.ニードル型生体顕微鏡と冠潅流制御装置を併用することによって、心筋虚血の好発部位である心内膜側の微小循環を冠動脈圧制御下に観測することに世界で初めて成功した。
2.冠動脈の血行動態を変化させると、心外膜側だけでなく心内膜側の細動脈においても速い時間応答で血管径、血流が変化することが明らかになった。
3.冠血行行動態を変化させた時、剪断応力に対する血管内皮由来一酸化窒素の時間的応答は従来の知見よりも速く、拍動的心筋血流と心筋エナ-ジェティクスの調節に寄与しうることが示された。
4.心筋エナ-ジェティクス、心筋細胞内分子機構にリンクした心収縮性の新たな指標が提案され、今後、この指標を利用しつつin vivoの心血管系における冠循環と心筋エナ-ジェティクスの相互連関を包括的に理解するためには心拍出の慣性力が心筋機能に及ぼす影響も考慮することの重要性が示された。
5.心筋内微小冠血管の血流、血圧、血管径が従来の知見よりもさらに微小な組織レベルで冠血行動態に鋭敏に反応して速い時間応答で分子レベルで調節されることが明らかになり、計測システムをさらに高分解能化させると同時に代謝パラメータも同時に評価することの必要性が示された。この問題については川崎グループを中心にアムステルダム大グループの協力を得て組織蛍光の評価法の検討を行うことで見通しはよいと考えている。
