肺癌の急増における経気道性因子、特に喫煙とアスベストの役割に関する病理学的研究

研究概要

近年の一般人肺癌の急増にアスベスト(AB)がどの程度関与しているかを検討するため,1950,70,90年代の原発性肺癌および70年代の転移性肺癌例の肺内AB量を定量し,各年代間で比較した.さらに70年代において原発性と転移性とで比較した.
原発性肺癌は,50年代8例(腺癌(Ad)+扁平上皮癌),70年代48例(Ad),90年代54例(Ad).70年代の転移性肺癌は18例.AB沈着量の測定は,低温灰化法による組織の灰化,および偏光による光学顕微鏡的観察にAB小体数の計測によった.
AB小体数(本/乾燥g)の平均値は,50年代559,70年代1804,90年代563となり,70年代での突出が目立った.原発と転移との比較では,男性において,原発性/転移性=2129/703と有意に原発性肺癌で高かった.70年代から90年代にかけて,AB量が大きく減少したことから,近年の肺癌の急増はABでは説明できない.しかし70年代症例で,原発性肺癌が転移性肺癌に比べ有意に高かったことは,一般の肺癌においてもABが一定の役割を果たしていることを示唆する.
さらに,90年代の肺がん(腺癌)において,喫煙・アスベストの混合曝露と遺伝子変化(p53変異と全染色体腕のLOH)や分化度との関係を検討した.今回は,非喫煙者分(n=46)を報告する.ABゼロ群(n=21),AB軽度群(0<AB<1000)(n=14), AB高度群(1000=<AB群)(n=11)での腺癌の分化度は各々,well/mod./poor.=6/11/4,6/8/0,4/7/0であり,高分化や中分化が多かった.FAL値は各々,0.17,0.07,0.13でABの量に関わらず、喫煙者の平均(0.22,n=66)より低かった.p53変異の頻度は3/19,4/14,5/11で,AB高値群で有意に高かった.結論として,非喫煙者の肺癌ではAB沈着量によらず高・中分化が多く,LOHの頻度は低い.しかし,p53変異はAB高値群では,有意に高い.
