研究概要

現在でもマラリアは世界的規模で流行しており、人的及び経済的損害を与える世界最大の寄生原虫感染症である。世界的視野に立ってマラリア制圧の戦略を練るためには、マラリアの診断・検出法の開発は不可欠である。なぜならば、現在でもマラリアの診断には患者の血液検体をギムザ染色し、顕微鏡下で検索するという古典的な方法が取られており、正確な診断には熟練を要する。全世界で一日あたりのマラリアの感染者数は70万人以上であり、流行地では莫大な数の被験者に対して検査技師の数が追いつかないのが現状である。本研究は、顕微鏡法にかわりうる簡便で確実な診断法を開発することを研究目的とする。
我々はマラリア原虫の18S rRNAの配列をターゲットとし、4種のヒトマラリア原虫を識別できる診断法(Microtiter-plate hybridization(MPH))を開発し、日本国内の327名の輸入マラリア患者の血液サンプルを用いてDNA診断を行った結果、マラリア陽性は226名、陰性は101名であった。原虫種別の内訳は、熱帯熱マラリア118例(36.1%)、三日熱マラリア83例(25.4%)、卵形マラリア16例(4.9%)、卵形マラリアvariant6例(1.8%)、及び四日熱マラリア3例(0.9%)で、複数の原虫種の混合感染例は全く見出されなかった。本診断法は日本の輸入マラリア患者の確定診断には向いてるものの、マラリアの流行地では電源の供給等設備の不足により我々の診断法を活用することは難しい。そこで新しい等温遺伝子増幅法(Loop-mediated Isothermal Amplification(LAMP))を用いて流行地で遺伝子の増幅装置を必要としない新しいLAMP法を開発した。本法はマラリア原虫の18S rRNA上に4つのプライマーを設計し、65℃でマラリア原虫に特異的な反応生成物を得ることができた。本法は従来のDNA診断法に用いる遺伝子増幅装置を必要としない、反応時間は極めて短い特徴を持っている。我々は微量の血液サンプルを用いて検出可能な改良法と検出感度を従来のものよりあげるための反応条件(プライマーの設計、酵素及び試薬の添加量等)を検討し、本LAMP法で熱帯熱マラリア原虫のDNAが増幅できたことを確認した。我々はこのLAMP法をマラリアの流行地で使用する計画を立てている。
