研究概要

手術侵襲により術後の免疫抑制が来されるが、そのメカニズムはよく知られていない。今回の研究の目的はmonocyte細胞表面のToll-like receptor(TLR)2及びTLR4の発現量と、それぞれのアゴニストに対する反応性が術前後に変化をするのかを明らかにすることである。対象は消化器疾患の手術を施行された患者とした。それぞれ術前後のperipheral blood mononuclear cells (PBMC)を抽出し、monocyteのTLR2,TLR4,inducible nitric oxide synthase (iNOS)の発現を測定したところ、TLR2及びTLR4の発現量は、共に術直後有意に低下し、術後1日目及び3日目にそれぞれ最低値をとることが明らかとなった。またTLR-2及びTLR4のアゴニストであるMacrophage-activating lipopeptide-2(MALP-2)又はlipopolysaccharide(LPS)にてPBMCを刺激し、産生されるTNFα及びIL-6の産生量を測定。MALP-2で刺激後のTNFα及びIL-6の産生量は、共に術直後に術前より有意に低下し、術後1日目に最高値を示した。LPSで刺激後のTNFαの術直後に有意に低下し、術後3日目に最大値を示した。PBMC中のiNOS発現量は術後に有意に増加していた。
この研究によりTLR2及びTLR4の発現量は手術侵襲により低下している事が示された。さらにそれぞれのアゴニストによるサイトカインの産生量は過性に抑制され、その後PBMCの活性化を通じて、直ちに増加していることが示された。この研究は手術侵襲後に於ける自然免疫システムの調整に対する、新しい知見を示している。
