吸収性材料による脊椎固定術に関する実験的研究

研究概要

当該研究の目的は、不安定脊柱に即時安定性を与え、しかも骨癒合成立時には生理的骨組織の剛性を持つ脊柱再建法として吸収性材料をもちいた椎体間ケージ,ならびに人工椎間板の初期固定のための吸収性材料による固定ピンの開発とそれらの有用性の確認・検討である.
研究1.高強度生体活性吸収性ケージを用いた椎体間固定に関する実験
未焼結のハイドロキシアパタイトとポリ-L-乳酸1を均一に混合し,特殊な圧縮形成(鍛造整形)により,羊腰椎用の高強度の生体活性・生体内吸収性椎体間ケージ(以下HA/PLLAケージ)を作成した.成羊26頭にHA/PLLAケージを用いて一椎間の後方進入腰椎椎体間固定を行った.8頭に自家腸骨ブロック,8頭に局所海綿を充填したカーボンケージ,10頭に局所海綿を充填したHA/PLLAケージを椎体間に各2個移植した.チタン製椎弓根スクリューによる内固定を併用した.120日後に屠殺し内固定金属を抜去した後,単純X線撮影,冠状断CT撮影を行った.またケージ周辺の状態を組織学的に検討した.HA/PLLAケージの骨癒合はカーボンケージより良好で,周辺骨との間の繊維性組織の介在は少なかった.前後屈・左右モーメントを負荷し2方向写真撮影で椎間可動域,剛性値を計測した.HA/PLLAケージによる再建椎間は自家骨置換と同様の椎間剛性を呈した.
研究2.吸収性ピンによる人工椎間板の初期固定性に関する生体力学実験
三次元編み物構造をした人頚椎用人工椎間板の初期固定のための吸収性ピン(以下HA/PLLAピン)を作成し,人死体頚椎のC5-6椎間で椎間板を切除し,非損傷椎,人工椎間板置換,腸骨移植,腸骨移植+前方プレート固定の順に力学試験を行った.試験は前後屈,左右側屈,左右軸回旋のモーメントの負荷試験とした.吸収性ピン使用による人工椎間板の初期固定は良好で,非損傷椎に近似した椎間剛性を呈した.また人工椎間板置換モデルの可動域は腸骨移植,腸骨移植+前方プレート固定より良好な可動性を示した.
