研究概要

本研究はゲノム多型解析などゲノム情報解析技術を用いて、HIV-1感染感受性やエイズ病態進行等の個人差を決定する宿主因子を解明し、エイズ制御のための新たな治療標的を見出すとともに、個人差に対応した緻密で適切な予後診断および治療指針の策定を目指す。現在までにHIV-1感染者ならびに非感染者のゲノム多型を検討し、以下の知見を得た。1.HIV-1の主要なコレセプターCCR5のアジア人特異的欠失変異893(-)を持つCCR5は、粗面小胞体から先への細胞表面への輸送効率良く進行せず、HIV-1感染効率が低下することを見出した。2.CCR2の64番目のアミノ酸がバリンからイソロイシンに置換した多型CCR2641のホモ接合がHIV-1に繰り返し暴露されながらも感染を免れているアジアの人々の中に濃縮されていること、64番目がイソロイシンに置換したCCR2のスプライスバリアントの一つCCR2A 64Iは、バリンのものより細胞内での安定性が向上し、HIV-1の主要なコレセプターCCR5の細胞表面発現を阻害することにより感染抵抗性に寄与すること、を見出した。3.IL-4はCCR5の発現を低下させる。 IL-4の転写を上昇させるプロモーター内の多型IL-4-5897がフランス人HIV-1感染者集団においてエイズ発症の遅延と相関すること、欧米人とはIL-4-5897の頻度が大きく異なるタイにおいてもこの多型がHIV-1感染者の未治療期間における死亡率を低下させること、を見出した。4.CCR5の生理的リガンドRANTES(CCL5)はCCR5に結合してHIV-1の細胞侵入を阻害する。RANTESのプロモーター部の多型RANTES-28は、RANTESの転写量を上昇させる。この多型がタイにおいてやはりHIV-1感染者の未治療期間における死亡率を低下させることを見出した。
