大腸癌の悪性度評価における細胞形質発現と遺伝子異常の関連

研究概要

これまでの研究で、胃型(human gastric mucin)、腸型(MUC2、CD10)のマーカーを用いた形質発現と大腸上皮性腫瘍の発育進展様式と悪性度および遺伝子異常の関連を解析し以下のことが判明し、これらの結果を基礎としてさらに解析を進めてきた。
1)Adenoma-carcinoma sequenceとde novo発癌経路では形質発現に違いがある。
2)絨毛性腫瘍や粘液癌、炎症性腸疾患に合併した癌では胃腸混合型形質が多い。
3)CD10の発現する癌では肝転移やリンパ節転移が高率であり、CD10発現は高悪性度の指標である。
そして、胃腸混合型形質が高率にみられる絨毛性腫瘍や炎症性腸疾患に合併した癌において、マイクロサテライト不安定性(MSI)の解析をhMLH1やMSH2を用いた免疫染色やマイクロサテライトマーカーを用いたPCR法により、混合型形質とMSIの解析し混合型形質を示す大腸癌ではMSIの関与があることが示唆された。若年者大腸癌とMSIの関連がしばしば報告されているが、若年者大腸癌では混合型形質の癌を高率に認め、hMLH1の免疫染色により混合型形質発現とMSIの関連が示唆された。
また、大腸癌の肝転移には既報告と同様にCD44v6、TGF-α、VEGF発現が関連があることが示されたが、CD10発現とはそれらの因子の関連は認められなかった。
