研究概要

1.AML患者3人に対するWT1ワクチンの長期投与による完全寛解の持続
2001年に、WT1ワクチンの安全性を評価するために、第I相臨床研究を開始した。天然型WT1ペプチド(CMTWNQMNL)あるいは、改変型WT1ペプチド(CYTWNQMNL)をモンタナイドアジュバントとともに、0.3mg/body、3人→1.0mg/body→3人、3.0mg/body、3人とdose-upしながら、2週間毎に計3回、皮内投与し、副作用と、臨床効果や免疫学的効果を評価した。
AML12人のうち、8人が臨床効果を評価しうる病変である微小残存白血病細胞(骨髄や末梢血のWTlmRNAレベルが高値)をもっていた。この8人のうち4人で、WT1ワクチン3回投与内に、WTlmRNAレベルが低下し、臨床効果が見られた。この4人のうち3人が(改変型WT1ペプチド3.0mg/body投与)が現在まで、5年〜5年2ヶ月にわたって、WT1ワクチンが継続投与されており、完全寛解が持続している。
2.白血病の頻縁疾患である骨髄異形成症候群に対するWT1免疫療法
WT1特異的CTLをゆっくりと誘導するための新しい第I相臨床研究を開始した。改変型WT1ペプチド5μg/body、3人→15μg/body、3人→50μg/body、3人とdose-upしながら、2週間毎に、計3回投与し、安全性を評価した。副次的に、臨床効果と免疫学的効果を評価した。5μg/body投与された3人のうち2人に臨床効果が見られた。1人では、WT1ワクチン投与後、白血球が徐々に低下し、臨床効果が出現した。この患者では、ごく少量のWT1ペプチドの投与にかかわらず、WT1特異的CTLが誘導されており、しかもEffectorタイプとMemoryタイプの両タイプのWT1特異的CTLが誘導されており、理想的な免疫応答が起っていた。
