研究概要

(本研究の概要)
研究では透析腎やvon Hippel-Lindau病患者腎に見られる腎細胞癌早期病変や前癌病変と考えられる微小病変をlaser capture microdissection(LCM)を用いて切り出し、正常部や腫瘍組織cDNAとcomparative cDNA microarrayを行い、遺伝子発現プロファイルを行うこと、およびchromogenic in situ hybridization(CISH)によって明視野下で形態とともに染色体数や転座を検討することを目的とする。
(平成15-16年度の成果)
1.検体の収集;研究の対象とする腎細胞癌症例を選択し、患者本人あるいは家族から、切除された検体(腎臓;ホルマリン固定パラフィンブロック)または新鮮凍結材料を研究に用いることについてのインフォームドコンセントを得た。臨床および病理学的情報を整理し匿名下を行った。
2.LCMおよびmicroarray:今回検索しえた透析腎癌症例7例およびvon Hippel-Lindau病症例2例では凍結ならびにパラフィン包埋組織からのLCMではmicroarrayに適したcDNAを得ることはできなかった。現在、保存状況の改善、検体作製過程の条件検討を行っている。
3.CISH;臨床的サイズに達した乳頭状腎癌既得検体5症例を用いて7、17トリソミー検出のための系を確立した。これをもとに乳頭状腎癌を発生した透析腎癌症例2例の単純性嚢胞、異型嚢胞、腺腫で検討をしたが、こうした染色体の異常は見いだし得なかった。現在、症例数を増やして検討中である。
4.この過程で腎癌および他腫瘍での17番染色体数とHER2遺伝子コピー数を検討する研究も並行して行った。乳房外Paget病で30%程度の症例でHER2遺伝子のコピー数増加を見た。
