研究概要

1.EBウイルス関連NK/T細胞増多症の疾患スペクトラム:本症の診断拠点施設としての役割を担ってきた。集積例の解析からそのスペクトラムを明確にし、悪性転化群の特徴を明らかにした(Arch Dermatol 2006,Eur J Dermatol 2006).皮膚リンパ腫登録制度を発足させ、EBウイルス関連リンパ腫の実態把握を開始。
2.非侵襲的診断法および細胞内遺伝子発現解析:痂皮を用いた非侵襲的EBウイルス潜伏感染診断法を開発し(J Microbiol Methods 2007),国際特許申請中(PCT/JP2006/317851)。他のヘルペスウイルス感染にも応用し、ウイルス遺伝子産物とともに細胞内シグナルを解析(Eur J Dermatol投稿中)。
3.ウイルス再活性化の証明と潜伏感染パターン変化:重症型では、病変内におけるEBウイルス再活性化シグナル(BZLF-1 mRNA)が明らかになった。一方、軽症の種痘様水疱症では、再活性化ではなく、皮膚病変内での潜伏感染IIへの移行がみられた。
4.EBウイルス感染細胞株解析:樹立EBウイルス感染NK/T細胞株にて,PMAおよびある種のサイトカインによって潜伏感染から溶解感染への誘導ができた(論文準備中)。
5.宿主免疫応答解析:ウイルス遺伝子発現と反応性CTLを解析して本症の主たる病態を明らかにした。
6.治療への取り組み:腫瘍抗原に対する免疫応答誘導を確認し(Int J Cancer 2007),同時に免疫機構からの回避現象を解析した(Cancer Immunol Immunother 2008).EBウイルス感染細胞がHDAC阻害薬に感受性があることを見出した。EBウイルス関連疾患における免疫・薬物療法展開にとって貴重なデータを得た。
7.EBウイルス関連リンパ腫を含めた診療ガイドラインを作成した。
