研究概要

疫学的検討から、NSAIDsの大腸癌の予防薬としての有効性が示唆されているが、我々は、その標的分子cyclooxygenase(COX)のうちの誘導型のCOX-2が大腸腫瘍で高発現しており、腫瘍細胞増殖、抗アポトーシス、腫瘍血管新生、腫瘍の浸潤転移、腫瘍免疫抑制など癌の発生進展において重要な働きをしていることを明らかにしてきた。本検討では、癌治療におけるCOX-2阻害剤の有効性を明らかにするため、マウスの皮下移植腫瘍に対し、5mm以上の大きさになってから、治療薬の投与を開始するモデルを用いた。COX-2阻害剤は単独では、固形腫瘍の増殖には有意な抑制効果を示さず、5FUは20mg/kgで有意な腫瘍増殖抑制効果を呈したが、COX-2阻害剤と5FUを併用した場合、5FU単独投与に比べ、より強い抗腫瘍効果が認められた。COX-2阻害剤、5FUはそれぞれ単独でも有意な腫瘍内血管新生抑制効果を示し、5FUとCOX-2阻害剤併用は、さらに強い抑制効果を示した。腫瘍内VEGF産生に関しては、それぞれ単剤で有意な抑制効果を示すが、併用による作用の増強は認められず、併用時の血管新生抑制の増強にはVEGF産生抑制以外の機序が関わる可能性が考えられた。腫瘍組織内のIFN-γ濃度は、併用で5FU単独に比べ有意に増加していた。その他、IL-10、IL-12濃度、腫瘍浸潤免疫担当細胞のプロファイルには有意な変化は認められなかった。IFN-γノックアウトマウスでは上記の野生型で認められた併用投与による腫瘍増殖抑制効果および血管新生抑制効果は認めらなかったことから、IFN-γが血管新生抑制を介し抗腫瘍効果を発揮している可能性が示唆された。今回の検討より、COX-2阻害剤は抗癌剤と併用することにより、IFN-g産生誘導を介し、抗癌剤の抗腫瘍効果を増強し、癌治療においても有効な治療薬となる可能性が示唆された。
