研究成果の概要

希少胆膵疾患の網羅的解析により，胆管や胆嚢の乳頭状腫瘍（IPNB，ICPN）では，APC，CTNNB1，STK11の変異が，胆管内管状乳頭状腫瘍（胆管ITPN）では，CTNNB1, SF3B1, BAP1，BRCA1の変異が特徴的と考えられた．プロテオームデータによるpathway解析では，IPNB・ICPNと胆管癌では活性化pathwayは共通していたが，活性化の程度に違いが見られた．IgG4関連疾患を含む非腫瘍性疾患のリンパ球レパトア解析では，有意なドミナントクローンは検出されず，ポリクローナルを示唆する結果であった．また， B細胞や免疫グロブリンに関連したpathwayが活性化していた．

研究成果の学術的意義や社会的意義

希少疾患は頻度が低いため研究対象とするのが困難である．そのため，その病態解析は進まず，診断，治療法の進歩が遅れることが問題とされてきた．本研究では，胆膵領域の希少疾患を対象に最新の技術を用いた研究を行い，腫瘍性疾患では特徴的な遺伝子異常や，活性化している分子経路が明らかとなった．非腫瘍性疾患でも活性化している分子経路が明らかとなり，今後の診断や治療に関連したより臨床的な研究につながる結果が得られた．
