研究開始時の研究の概要

Telemedicine aims to slow the spread of infection, and to enhance the quality and cost of healthcare. However, its use in Japan is still limited. This research aims to increase the patients’ awareness and willingness to use a telemedicine service. It also seeks to explore the usability problems of a telemedicine system and create innovative solution/s that meet the needs of as many patients as possible. The research focuses on people with special needs, including elderly patients with chronic diseases. Participatory design approaches will be used to achieve our goals.

研究実績の概要

In the first stage of study, 110 outpatients with chronic diseases were contacted at the Kyushu University Hospital, and data were collected from them using face-to-face questionnaires. Group 1 and group 2 separately include 55 patients aged 40 to 64 years and 65 to 90 years, respectively. The survey aimed to evaluate awareness of telemedicine and identify patients' problems and needs to develop the service. The results of the survey revealed that the acceptance of telemedicine concept by age and gender was found to be greater among middle-aged adults (2.9, SD 0.7) and male patients (2.9, SD 0.6) than older adults (2.5, SD 0.8) and female patients (2.5, SD 0.9), and the difference in each aspect was significant (P=0.012, 0.024), respectively. Middle-aged adults (2.8, SD 0.6) were also found to be more willing to use the service in the future than older adults (2.5, SD 0.7), and the difference was significant (P=0.037). On the other hand, the limited use and slow growth of telemedicine among the patients are related to the following main issues: limited knowledge of the service, usability challenges of telemedicine technologies, and cultural factors. Regarding the development of telemedicine, key recommendations were made for creating a user-friendly telemedicine that meets the patients' needs and expectations.

今後の研究の推進方策

In the next stage of research, usability tests (UTs) will be carried out at the Kyushu University hospital with mid-aged and older adults (chronic diseases outpatients) from the hospital (get paid for their participation). The UTs will be conducted using the Telehealth Usability Questionnaire (TUQ) to determine the patients’ problems while preparing for online visits and using of telemedicine technology, as well as to identify the influence of usability on their emotional state. About 5 physicians from the hospital will be recruited to conduct the tests. In the tests, five elements will be examined including learnability, interface quality, reliability, interaction quality and satisfaction. Three stages will be evaluated in the test: pre-visit stage, visit stage and post visit stage.
