研究概要

がん根治を目的とした自己造血幹細胞移植に関連する5つの基礎的課題をとり上げ、それぞれについて以下のような成果を得た。
1.造血幹細胞の純化ならびに自己複製:ヒト幹細胞はCD34陽性細胞分画に濃縮されること、またマウス幹細胞はIL3とIL6の共存下でin vitro増幅が可能であることが示された。一方、EPO刺激下で赤血球生成を支持するマウスストローマ細胞株が確立され、op/opマウス由来線維芽細胞株のMーCSF産生欠損が明らかにされるなど、造血におけるストローマ細胞機能の基礎的研究の道が開かれた。
2.末梢血造血幹細胞:GーCSFを投与すると末梢血中の造血幹細胞が増加することが確認され、上記IL3、IL6とともに末梢血幹細胞移植への造血因子応用の期待が得られた。
3.移植患者の造血因子動態とストローマ細胞機能:各種造血疾患の骨髄ストローマ細胞を用いてその造血因子産生能の臨床的意義を明らかにした。また、血中動態の知見に基づいたGーCSF、EPOなどの移植後投与の臨床的有用性を確立した。
4.残存白血病細胞の検出:Ph^1陽性ALLのbcr-abl transcriptsを指標にPCR法の有用性を示唆した。
5.移植前処置法の開発:GーCSFの移植後投与の臨床的有用性が確立したことを背景として悪性リンパ腫、乳がんを対象とした大量のthioTEPAを主体とした前処置法、ならびに骨髄性白血病を対象としたGーCSF併用前処置法を開発し、その臨床検討に入った。
以上より、次年度はストローマ細胞と造血因子を用いたin vitroヒト造血幹細胞の選択的増幅法の確立、上記の2〜3のプロジェクトの臨床評価を最重点項目として研究を逐行していく予定である。
