ケラチン、インボルクリン、フィラグリン抗体を用いた口腔粘膜角化症の新しい診断法

研究概要	口腔粘膜の角化病変(良性悪性を含む)の免疫組織化学的検索により、正常粘膜において一般に、各種ケラチンサブユニット、involuclinは層的分布をとるが、分布するケラチンサブユニットが咀嚼粘膜上皮、裏装粘膜上皮、および機能粘膜上皮において異なることが判明した。良性角化病変においては、一部のケラチンサブニットの染色性の低下などは認められるものの、基本的な層的分布は乱れず、異型細胞の出現、悪性の程度に判い、層的分布は乱れ、細胞ごとの染色性が異なる様相を呈することが判明した。さらに本年度、新たにヒト口腔粘膜上皮のEGFレセプタ-の分布を免疫組織化学的に検索したところ、正常粘膜、乳頭腫の症例においては、基底細胞、傍基底細胞、下部棘細胞層の細胞膜に反応を認める。正あるいは錯過角症のみを呈する白板症においてはEGFレセプタ-の反応は認められないが、同じ白板症として臨床的には診断される病変のうち、上皮釘脚の伸長した症例では伸長した上皮釘脚部の傍基低細胞、棘細胞の細胞膜に陽性反応を認めた。しかしながら扁平上皮癌においては多くの症例でEGFレセプタ-の層的な反応は認めず、散在性に細胞膜での反応を認めるのみであった。さらに同じ症例において連続切片あるいは一部はEGFレセプタ-との二重染色法を用いて、Proriferatiog cellnucleor cutigen(PCNA)を証明したところ、基底細胞層におけるPCNAの陽性率は伸長した上皮釘脚を持つ白板症の症例で高く、過角化症をのみ示す白板症や乳頭腫、正常粘膜では低い。扁平上皮癌の症例においては、PCNA陽性細胞は基底細胞層や傍基底細胞層に相当する部分のみならず、さらに上層に存在する細胞にも認められ、全上皮層での比較では陽性率が最も高った。またPCNAに陽性を示す細胞はEGFレセプタ-の反応を示さないことが判明した。
